Head contamination on the LTO-8 narrow-track array, or a worn head assembly nearing end-of-life. We run an ITDT extended head-health test to distinguish contamination (fixable with cleaning) from wear (requires head replacement).
IBM’s LBP is a per-block data-integrity check that detects write corruption in real time. When ITDT logs LBP failures during Veeam or Spectrum Protect writes, the root cause is almost always a head calibration issue or a firmware bug. We diagnose to the correct tier before quoting.
The library reports ‘destination element invalid’ or ‘source element empty’ even when cartridges are loaded. The picker arm has lost its slot-teach map. We re-map element addresses via SCSI Medium Changer commands and re-teach the arm position.
The drive does not appear on sg_scan (Linux) or Windows Device Manager after a power cycle. Root cause is typically the mini-SAS HD connector pins on the drive or the interface board — not always the HBA (host bus adapter, the SAS card in the server).
The drive is stuck at the 0xC2 self-test code, LEDs cycling without reaching the ready state. Serial firmware reflash with the correct IBM recovery image restores function. Turnaround 1–2 working days.
Enterprise-scale libraries report ‘park position lost’ or accessor calibration alerts. TS4500 service requires on-site attendance at your data centre — we coordinate the visit via WhatsApp.
